Instructions for 4-H Youth Volunteers:
You will need to fill out the enrollment form below and download the medical form. A parent will need to sign it.
The Liability Release form for the Barn also needs to be downloaded below and signed by a parent.
Bring these two forms and either a check made out to Westwind 4-H or cash for $15.00 when you come to the orientation/training.
One warning: The 4-H website does not support Safari ,so you will need to open it using the Firefox browser. If you use a PC there should be no problem.
